Referral Process
See the insurance plans we are in-network with here. You are welcome to use your own fax form, but if you need one, here is our form: Referral for Medical Nutrition Therapy. Please send over demographic information, lab results, progress notes, and any other information that will help with treatment. We will contact the client and schedule them.
